Common types of amylase detection and its clinical significance

1. Increase of serum amylase:

(1) Acute pancreatitis: it is most common in acute pancreatitis. After the onset of the disease, the activity began to increase at 2-12h, peaked at 12-72h, and returned to normal at 3-4 days. Although the increased level of amylase activity is not necessarily related to the degree of pancreatic injury, the greater the degree of increase, the greater the possibility of acute pancreatitis. In case of suspicion of acute pancreatitis, amylase should be continuously monitored and combined with other biological enzyme tests, such as pancreatic lipase, trypsin, etc.

(2) Acute abdomen: other acute abdomen also can cause amylase activity to rise.

(3) Chronic pancreatitis: the activity of amylase can be slightly increased or decreased in chronic pancreatitis, but it has no great significance in diagnosis.

(4) Pancreatic cancer: amylase activity can also be increased in the early stage of pancreatic cancer.

2. Rise of urine amylase:

Biological enzyme preparation reminds you that the amylase in the blood can be filtered by the glomerulus. When the serum amylase is increased, the amylase output in the urine will increase. The increase can be earlier than the blood amylase, and the decrease later than the blood amylase.

3. Amylase isoenzyme:

Serum amylase comes from pancreas, salivary gland and many other tissues, so when amylase activity increases, isoenzyme measurement is helpful for differential diagnosis.

When p-isozyme is increased or decreased, there may be pancreatic disease isozyme changes which may originate from salivary glands or other tissues.

The determination results of amylase are greatly influenced by the methods, and the reference values of different methods are different. It is necessary to know the determination methods and reference values used in order to make a correct diagnosis.
